By submitting my firearm to Bullseye, LLC, I have voluntarily requested that the aforementioned company perform modifications to my firearm that include, but not limited to permanent modifications or alterations. I am aware that any requested alteration and/modification may void my factory warranty, also be hazardous and/or may affect various safety systems, and never return to original condition. I am voluntarily requesting that these activities take place with the full knowledge of the dangers involve, which include mechanical failure, damage to personal property, bodily injury, and death. And I hereby agree to accept and assume any and all risks involved including injury or death. As consideration for services being provided, I hereby agree for myself, heirs, personal representatives and assign release, indemnify, hold harmless, and forever discharge Bullseye, LLC and its agents, employees, officers, principles and legal representatives of and from any claims, demands, debts, contracts, expenses, lawsuits, damages, and liabilities, of every kind of nature, whether known or unknown, in law or equity, that I ever had or may have, arising from or in any way related to the work performed on my firearm. bullseyellcsales@gmail.com
